This last Christmas, there was an ep of Doctor Who where the Doctor's hand got cut off. It is his hand in the jar (he grew another). And Jack is now different from the other boys, the Doctor is sort of behind the change.
You should watch the end of S1 Doctor Who, starting with The Empty Child, and then it will make MUCH more sense. I don't have the patience to download episodes, myself, so I got caught up on tv-links (http://tv-links.co.uk/listings/1/26). Watch 1x09-13 and the Christmas Invasion, then watch Torchwood, then watch season 3 (or at least the last three episodes) of DW.
The hand is a reference to the very first Tennant episode, "The Christmas Invasion", with the Sycorax. You might want to chase it down. They give a synopsis of The Hand again in the series 3 WHO episode "Utopia" as well, as it becomes an important plot point...
